Obtain Driver Exchange Information

Follow the following steps to obtain your information online. Please allow 3-5 days for the information to update on the website: 

• Click here.
• Select the State of Illinois from the list of states.
• Select the Crystal Lake Police Department from the agency drop down box. 
• Click start search. Search using one of three options:
  • By Report Number
  • By Last Name and Date of Incident
  • By Last Name and Location
• Click search. 
• If the report is complete, both the accident report and free driver exchange will pop up.
• Requestors will need to create an account on the site to show confirmation that they are an involved party member.
• Once logged in, you can add the driver's exchange report or accident to the cart to download.
