The City of Crystal Lake values the safety and reliability of established businesses in the community. Annual Massage Licenses expire on April 30th of each year. Please contact Victoria Wojcik to submit the appropriate documents.

 

License Renewals

Please submit your updated application with the required documents included in the Massage License Application Instructions handout. Licenses are renewed at no application fee.

Please submit an updated lease agreement and proof of insurance with your application.

Please submit a current photo, state-issued ID, and Massage License for each massage therapist employed in your establishment.

 Background Checks

Background checks are required for each new business owner or manager. Each background check is a $50 fee.

 

New Establishments

Please submit your completed application with the required documents included in the Massage License Application Instructions page.

Managers and/or owners require a background check through the City of Crystal Lake Police Department. Please fill out the Background Check Request Letter with your application.

 Fees

The fee for the Massage License is $50 per new business.
Each background check is an additional $50 fee.

Limited Use Permit

New massage establishments must abide by the City of Crystal Lake zoning requirements. A Limited Use Permit is required for establishments offering massage services.

A site inspection will occur to ensure compliance for the Limited Use Permit.
