Food Vendor Permits
The City of Crystal Lake requires annual permits for food vendors to operate outdoors within Crystal Lake City Limits. Please apply for your Food Vendor permit by creating a record and submitting all required documents through our EzApp Online Permit Portal.
Once the application and all appropriate documents are submitted, the City of Crystal Lake will contact the applicant to schedule an inspection with the Fire Rescue Department. Food stands do not require an inspection with the Fire Rescue Department.
A Food Vendor permit has an annual fee of $50, and full permit approval is valid through December 31st of the current year.
The City of Crystal Lake Food Vendor requirements and conditions are found in Chapter 385 Article V of the City Code.

When applying for the Food Vendor Permit, please include the following documents:
1. Completed Food Vendor Application Form
2. The Certificate of Insurance
3. Most recent McHenry County Health Department Approval
